The structure of the three-aisled basilica on the Caelian Hill dates to Pope Paschal I (817-824), including the magnificent mosaic and splendid lost liturgical furnishings. Although it has been the seat of an archdeaconry since the 1120s, no significant building or furnishing work seems to have been necessary during the High Middle Ages. Since the 16th century, several modern interventions inside, including the construction of a crypt in the 1950s, have erased all traces of the medieval period.
