According to Pompeo Ugonio's description of the building in c. 1590, the church had an entrance portico and the interior was divided into three naves; sixteen columns flanked the nave, presumably eight columns with nine arches on each side. In 1179, Pope Alexander III (1159-1181) consecrated a side altar dedicated to Mary. The pavement is given special mention by Ugonio, described as "artful with various stones", but is now lost. Until the 19th century, the design of a labyrinth was preserved in the centre of the floor and, again according to Ugonio, the apse was decorated with post-medieval paintings. To the left of the high altar was the 1295 altar of St Nicholas, donated by Cardinal Jacopo Colonna. What Ugonio saw was perhaps still the 8th century basilica with remnants of what must have been the liturgical furnishings of the High Middle Ages.
